Diabetes Prevention Program (DPP)

The Diabetes Prevention Program (DPP) is a lifestyle change program for people with pre-diabetes to reduce their risk of type 2 diabetes and improve their overall health. The mission of the DPP is to reduce the number of adults in Harris County diagnosed with Type 2 diabetes. The program targets adults over the age of 18 that have been diagnosed with pre-diabetes.

Pre-diabetes is identified when blood sugar levels are higher than normal but not high enough to be diagnosed as type 2 diabetes. Diabetes is a significant concern in the United States as it is the nation’s seventh-leading cause of death.

The Prevent T2 curriculum can be offered in English or Spanish and includes a participant’s guide and resources to successfully complete all sessions. All lessons are taught by certified Lifestyle Coaches along with a Registered Dietitian that will focus on teaching the participants new skills while encouraging them to meet their goals and keep them motivated. The program helps participants lose 5% -7% body weight and increasing physical activity to 150 minutes per week.

Can the program be taught in English and Spanish?

Yes, all sessions are available in English and Spanish.

Is the program free?

Yes, the program is offered at no cost to participants.

What are the eligibility requirements for the program?

Must have/be:

  • 18 years old or older
  • BMI above 25(BMI above 23 if Asian American)

Must meet one of the additional criteria:

  • Hemoglobin A1C level between 5.7-6.4% (check with your healthcare provider)
  • Previous clinical diagnosis of gestational diabetes
  • 5 or higher on the DPP risk test. Link here

Does not meet requirement for program:

  • Currently pregnant
  • Has been clinically diagnosed with type 1 or type 2 diabetes by health care provider

How long is the program?

The DPP program is a 24-week session that last 12 months. We will meet for one-hour:

  • Weekly for the first 16 sessions
  • Monthly for the last 8 sessions
Do you provide incentives for participants?

Yes! Throughout the sessions participants can receive different incentives correlating to each lesson such as pedometers, weight scales, workout bands, and raffles (I.e., Air Fryers)
